Yale Materials Handling - Minnesota, Inc. offers several forklift training classes throughout the year. Every year tens of thousands of injuries occur due to lift trucks in the workplace. Unfortunately, most incidents are due to a lack of safe operating procedures and insufficient forklift training.
According to OSHA (Occupational Safety and Heatlh Administration), the employers shall ensure that in order to operate a powered industrial truck the operator has completed an evaluation and forklift training course.
Refresher training is required by OSHA when:

Operator has been observed operating the forklift in an unsafe manner

Operator was involved in an accident or near-miss incident
-
Evaluation reveals the operator is not operating the lift truck safely

Operator is assigned to a different type of truck
-
Workplace changes in a manner that could affect safe operation

Evaluation of each operator's performance shall be conducted at least once every 3 years
